- Application for Permit with Itinerary - download standard form
- Amusement Device List - download standard form
- Certificate of Insurance (COI) (minimum of $1,000,000 liability per occurrence)
- Inspection Report (must include both the TN online Inspection Report and the supplemental inspection report from the inspector); Inspection Reports must be dated within three (3) month or 90 days from the permit application date
- Remit the permit application fee of $150 per company per fixed location
